Local Section & Student Chapter Forms

All OSA Chapters and Sections are required to submit an Annual Report to be eligible to participate in the Activity Grant, Excellence Award, Education Outreach Grant, and Traveling Lecturer Programs.

Below is a list of participation forms for Local Sections & Student Chapters:

• Local Section Annual Report Form - (Due May 31)

• Student Chapter Annual Report Form - (Due May 31)

• Activity Grant Final Report - (Due 30 days from the completion of your program/event)  

• Local Section Activity Grant Application - (Due Oct 31 and April 15)

• Student Chapter Activity Grant Application - (Due Oct 31 and April 15)

• Local Sections Excellence Award

• Student Chapters Excellence Award

• Local Section OSA Foundation Education Outreach Grant Application - (Due Oct 31 and April 15)

• Student Chapter OSA Foundation Education Outreach Grant Application - (Due Oct 31 and April 15)
